The fashion career of Betsey Johnson began after entering and winning the Mademoiselle Guest Editor Contest. She eventually became the in-house designer for the Manhattan boutique called Paraphernalia. She became a part of both Andy Warhol's underground scene and the youthquake fashion movement along with Lou Reed, Nico, Edie Sedgwick, and The Velvet Underground. In the year 1969, she was able to open a boutique that was called Betsey Bunky Niniat the Upper East Side of New York City. Her house model was Edie Sedgwick and she designed the clothing Sedgwick wore on her last film which was Ciao! Manhattan.

Johnson was able to take control of the fashion label called "Alley Cat" in the 1970s which was popular with rock 'n roll musicians. In the first year, her debut collection for Alley Cat was able to sell approximately $5 million. Because of this, she received the Coty Fashion Critics' Award in the year 1971.

In the year 1978, Johnson began her fashion line. Since her second collection did not fared well in the market, she was left with 3,000 pieces of clothing without enough funds to stage a fashion show in 1981. Johnson eventually opened a retail store in New York's SoHo area and designed the dress that Lisa Loeb was able to wear in the music video of her 1994 hit "Stay (I Missed You)."

Johnson eventually got inducted in the Fashion Walk of Fame in the year 2002. The bronze plaque showed one of her original sketches. She eventually expanded her line to include scarves, handbags, hats, and accessories.

Betsey Johnson became famous for her whimsical and feminine designs and has earned her net worth as the in house designer of the Manhattan Boutique Paraphernalia. She eventually took over the label Alley Cat, started creating her fashion line and set up numerous fashion stores all over the world.
