Mantooth began his acting career when he was discovered by talent agent Eleanor Kilgallen while performing the lead role of Gar in a New York theater production of Philadelphia Here I Come. He was honored with a Charles Jehlinger Best Actor Award for his role in the film.

He appeared on several TV series such as Adam-12; Marcus Welby, M.D.; McCloud; and Alias Smith and Jones. In 1972, he landed a role as Johnny Gage on the television series, Emergency!, becoming popular and staying five seasons.
