Josh Smith joined the John McEachern High's basketball team. During his senior year, he transferred to Oak Hill Academy and played an averaged of 22 points, 8 rebounds, 4 assists, 6 blocks, and 3 steals per game which the team led to 38-0 record.

He committed to play for Indiana University but he decided to forgo college and enter the NBA Draft, instead. Smith was selected by the Atlanta Hawks as the first round, 17th overall pick during the 2004 NBA Draft.

Smith broke the 500 career block mark on March 3, 2007 which makes him the youngest player who did it in the NBA history. In 2013, he signed a four-year contract with Detroit Pistons. While with the Pistons, he averaged 15.5 points, 6.9 rebounds, and 3.7 assists per game. After a year, he moved to Houston Rockets.
